We Utilize A Team Approach to Care:
Oversees all operations of the company, develops client care standards, caregiver training and orientation curricula, and reviews any client complaints to ensure proper resolution.
Oversees care quality, supervises all Services, oversees the scheduling and staffing departments, and supervises on-call staff.
RN, Therapy, HHA, PCA, CLS/Respite
Provide Services in the home. Professional, experienced and trained to meet the need of each client.
A clients designated one or more family members with whom M&Y CARE interacts about care issues, changes of condition, scheduling, questions, as well as billing issues.
Client’s Health Care Professionals
M&Y CARE clinical team works closely with physicians to develop a personalized treatment plan. By working together it ensures that patients receive the services and care the patient needs for the best clinical outcomes.
RN Care Coordinator
Assess client and family needs, develop and update plans of care, perform in home employee introduction and orientation, perform unannounced in-home care evaluations, conduct family care conferences and documenting significant client and client events.
Business Service Director
Serve as family’s main day to day contact over the phone and in person, supervise the performance of caregivers, and document important events related to client care.
